Services - Day Care Licensing
If you are interested in becoming a licensed day care provider,
you will need to download a number of documents that can be accessed by
clicking the link below:
Initial
Day Care Licensing Packet documents
- Each household member, age 13
years of older must complete #'s 6, 8, and 9.
- Each household member, age 13
years or older must complete #7 if they intend to help or substitute
at your day care 30 days or more in one year.
- Individuals other than household
members who are 13 years old and older who intend to help/substitute
for you, must complete #'s 6, 8, and 9. In addition, #7 if they will
be helping or substituting 30 days or more in one year.
You may submit your Application,
Applicant Background Studies and the Day Care licensing fee of $50 to:

You will receive a letter informing you of the next scheduled Day Care
Orientation Meeting.

You may submit your completed relicensing
paperwork with the $100 Day Care relicensing fee to the above address.
This must be submitted at least 60 days prior to your license expiration
date.
