Information for Inspectors, Installers and Designers

The form below is a county required to obtain a permit for new or replacement of onsite sewage treatment systems. Please print the form, complete it, and bring it to the Land & Resource Management Department at the Douglas County Courthouse to obtain the SSTS permit.

One of the following forms must be returned to Land and Resource Management upon completion of a septic inspection:

SSTS certified or conforming
SSTS non-certified or non-conforming
